Staging of mesothelioma is based on imaging studies such as x-rays, CT scans, and MRI scans. The treatment and outlook for patients with mesothelioma largely depends on the stage (extent of spread) of their cancer. Since pleural mesothelioma occurs more commonly and has been studied the most, it is the only mesothelioma for which a staging classification exists.</p> <p>The staging system most often used for mesothelioma is the Butchart system. This system is based mainly on the extent of the primary tumor mass, and divides mesotheliomas into stages I through IV.</p> <p>Butchart Staging System</p> <ul><li><strong>Stage I:</strong> Mesothelioma is present within the right or left pleura, and may also involve the lung, pericardium, or diaphragm (the muscle separating the chest from the abdomen) on the same side.</li></ul> <p> </p> <ul><li><strong>Stage II:</strong> Mesothelioma invades the chest wall or involves the esophagus (food passage connecting the throat to the stomach), heart, or pleura on both sides. The lymph nodes in the chest may also be involved.</li></ul> <p> </p> <ul><li><strong>Stage III:</strong> Mesothelioma has penetrated through the diaphragm into the peritoneum (lining of the abdominal cavity). Lymph nodes beyond those in the chest may also be involved.</li></ul> <p> </p> <ul><li><strong>Stage IV:</strong> There is evidence of distant metastases (spread through the bloodstream to other organs).</li></ul> <p>Another staging system has recently been developed by the American Joint Committee on Cancer (AJCC). This is a TNM system, similar to staging systems used for most other cancers. T stands for tumor (its size and how far it has spread to nearby organs), N stands for spread to lymph nodes and M is for metastasis (spread to distant organs).</p> <p>TNM Staging System</p> <ul><li><strong>Stage I:</strong> Mesothelioma involves the right or left pleura. It may also have spread into the lung, pericardium, or diaphragm on the same side. It has not yet spread to the lymph nodes.</li><li><strong>Stage II:</strong> Mesothelioma has spread from the pleura on one side to the nearby peribronchial and/or hilar lymph nodes next to the lung on the same side. It may also have spread into the lung, pericardium, or diaphragm on the same side.</li></ul> <p> </p> <ul><li><strong>Stage III:</strong> Mesothelioma has spread into the chest wall muscle, ribs, heart, esophagus, or other organs in the chest on the same side as the primary tumor, with or without spread to subcarinal and/or mediastinal lymph nodes on the same side as the main tumor. Subcarinal nodes are located at the point where the windpipe branches to the left and right lungs. Mediastinal lymph nodes are located in the space behind the chest bone in front of the heart. Mesotheliomas with the same extent of local spread as in stage II that have also spread to subcarinal and/or mediastinal lymph nodes on the same side are also included in stage III.</li></ul> <p> </p> <p> </p> <ul><li><strong>Stage IV:</strong> Mesothelioma has spread into the lymph nodes in the chest on the side opposite that of the primary tumor, or directly extends to the pleura or lung on the opposite side, or directly extends into the peritoneum, or directly extends into organs in the abdominal cavity or neck. Any mesothelioma with evidence of distant metastases (spread to other organs through the bloodstream) or spread to organs beyond the chest or abdomen is included in this stage.</li></ul> <p>Although the recently developed TNM classification is the most detailed and precise, the original Butchart staging system is still used most often to describe the spread of pleural mesotheliomas. Edie Falco – the breast cancer was diagnosed in 2003; this soprano star secretly battled against this dreaded disease and was able to survive.<br /><br />2. Melissa Etheridge – her breast cancer was discovered in 2004 and at present, she is doing quite well.<br /><br />3. Gloria Steinem – a lump on her breast was discovered in 1984; she was an activist and because she is a natural fighter, she was able to combat breast cancer.<br /><br />4. Kate Jackson – it was in 1987 and 1989 when Kate battled against this disease. She was a star in Charlie’s Angels<br /><br />5. Important drops were seen in the three leading causes of cancer deaths for men: lung, prostate and colorectal.<br /><br />In women, death rates from colorectal and breast cancer was down, while the increase in lung cancer deaths slowed quite noticeably.<br /><br />Good news to be sure, and a sign that new therapies, early diagnosis and continued research are making a mark against this terrible disease.<br /><br />Cancer prevention also makes a marked impact on cancer diagnosis, and is the motivation behind ten recommendations made last year by the American Institute of Cancer Research and presented at 2008's annual meeting of the American Dietetic Association.<br /><br />Written by an international team of scientists, including Walter Willett, MD, DrPH, an epidemiology professor who heads the nutrition department at the Harvard School of Public Health, the ten lifestyle tips were reviewed by Karen Collins, MS, RD, CDN, the nutritional advisor for the American Institute for Cancer Research. Both Collins and Willett discussed the suggestions with attendees at the American Dietetic Association's annual meeting that's just wrapped in Chicago.<br /><br />Take a look at the recommendations for yourself… common sense really, but absolutely worth including in your life, especially if you have high risk factors for cancer.<br /><br />1) Be as lean as possible, but not underweight. Collins suggests not just checking the number on the scales, but also measuring your waist to get a feel for your abdominal fat. Men should have a waist measurement no larger than 37 inches; women's waists should be 31.5 inches or less.<br /><br />2) Be active for at least 30 minutes every day on most days of the week. If you're out of shape or very sedentary, talk with your doctor first. Then start slow and build gradually over time. And contrary to popular belief, you can break up your workout - 10 to 15 minute sessions, twice a day are just fine according to Collins.<br /><br />3) Avoid sugary drinks and consumption of energy-dense foods. No one is saying these foods (or their additives) cause cancer, but they do add extra calories to your diet, and can sabotage your healthy weight over time. A treat now and then is fine, all the time can really blow your calorie budget.<br /><br />4) Eat a variety of vegetables, fruits, whole grains and legumes. You'll want to try to add a lot of different colors (deep green leafy veggies, blueberries, etc.) to your meals. Most Americans, says Collins, are stuck in a rut of eating the same three vegetables over and over. Try something new, you just might like it and be helping your body at the same time!<br /><br />5) If you drink at all, limit alcoholic drinks to two a day for men and one for women. To do this, you'll need to keep an eye on the bartender as according to Collins, drinks can have a higher alcohol content than expected. Willett was quick to caution that the pros and cons of moderate drinking is something that women should consider carefully, weighing the heart benefits against the increased breast cancer risk from drinking.<br /><br />6) Limit red meats (beef, pork, lamb) and avoid processed meats. Good as they may taste, you'll want to keep your red meat intake to 18 ounces a week, says Collins. She suggests choosing chicken, seafood, or legumes in place of red meat, and encourages moderation rather than eliminating these meats entirely from the diet.<br /><br />7) Limit consumption of salty foods and foods processed with sodium. You'll want to try and keep your sodium intake under 2,400 milligrams a day. To add flavor to food, use herbs and spices instead, says Collins. She points out that processed foods account for a large part of sodium intake today - so don't worry so much about the salt you add when cooking or eating, rather read the labels of the foods you buy.<br /><br />8) Don't use supplements to protect against cancer. This is a recommendation that is likely to be reviewed, but for now the experts are cautioning against taking a supplement purely for cancer protection benefits. It's not that supplements are bad, but the science doesn't support (except in the case of vitamin D) the idea that these substances can protect against cancer.<br /><br />9) Mothers should try to breastfeed babies exclusively for up to six months and then add other foods and liquids. If this is an option for you, it's a good idea and provides the baby with many healthy benefits.<br /><br />10) After treatment, cancer survivors should follow the recommendations for cancer prevention. This includes patients who are currently undergoing treatment, as well as those who have finished their therapy and are being monitored. Once you've beaten your cancer back, you need to do all you can to keep your body in top form - healthy and strong enough to resist anything that comes your way.<br /><br />You need to keep in mind that while these tips are sensible suggestions they are only intended to reduce (not eliminate) your risk of cancer. In organizing the factors involved, the prognosis or the most likely outcome of the disease will be identified. The doctor can also provide the patient with the best treatment guide for the disease.<br /><br />Stage 0. In this stage, the breast cancer is non-invasive. It just simply means that there is no sign of non-cancerous abnormal cells or cancerous cells getting out of the part of the breast where they started. These cells have not yet invaded other neighboring normal tissues in the breast.<br /><br />Stage 1.<br /><br />In this stage, the breast cancer is said to be invasive. The cancer cells are already breaking out of the part of the breast where they started and they have already invaded neighboring normal tissues. But in Stage 1, the lymph nodes are not affected or involved and the tumor measures up to 2 centimeters.<br /><br />Stage 2.<br /><br />Stage 2 is described as invasive breast cancer and is subdivided into two categories: Stage 2A and Stage 2B. In Stage 2A, there is no tumor in the breast but a 2 centimeters or less tumor can be found scattered in the axillary lymph nodes, which is the lymph node found under the arm.<br /><br />Another case of Stage 2A is that, the tumor is larger than 2 centimeters but not more than 5 centimeters and has not spread in the axillary lymph nodes. In stage 2B, the tumor is also larger than 2 centimeters but not more than 5 and has already spread to the axillary lymph nodes or if the tumor is larger than 5 centimeters, the tumor has not yet spread the axillary lymph nodes.<br /><br />Stage 3.<br /><br />Stage 3 is described as one of the invasive breast cancer stages and is subdivided into 3 categories: Stage 3A, Stage 3B and Stage 3C. In Stage 3A, there is no tumor in the breast but it is found in the axillary lymph nodes or the lymph nodes near the breast bone. The tumor has the size of less than 5 centimeters or more and they are clustered together or they may stick to other structures.<br /><br />In Stage 3B, the tumor may be of any size and found in the lymph nodes under the arm or near the breastbone, chest wall and/or skin of the breast, clustered together or sticking to other structures. In Stage 3C, there can still be no sign of cancer in the breast. The study is of particular importance to the 2.4 million breast cancer survivors, and to states like Texas, where over 12,000 women are expected to be diagnosed with the disease this year and over 25% of the population is going without health insurance.</p> <p>Published in the Journal of the American Medical Association on July 18th, the study focused on over 3,000 women successfully treated for early-stage breast cancer. Average age of participants was fifty-three, and ten percent died within the course of the study, mostly due to a recurrence of the disease. The cancer returned for approximately the same number of individuals in each group.</p> <p>Diets assigned to 1,537 of the participants exceeded the Recommended Daily Allowance (RDA) of five fruits and vegetables per day, and consisted of five daily vegetable servings, three fruit servings, sixteen ounces of vegetable juice, and thirty grams of fiber. A "serving" was to equal one-half cup, French fries and iceberg lettuce could not be counted, and only fifteen to twenty percent of calories could be derived from fat. Control groups were given educational literature on the importance of eating at least the RDA of fruits and vegetables.</p> <p>But the study is already undergoing criticism from healthcare professionals and researchers, many of whom believe the causes of serious diseases like cancer are multi-dimensional, and that studies focusing on a few macronutrients or food groups -- with few other controls -- may be unable to yield the intended data with accuracy. Previous studies on the link between healthy diet and the prevention of breast cancer have returned mixed results at best.</p> <p>Susan M. Nor did it control for environmental factors, or other variables associated with breast cancer, including obesity and exposure to certain chemicals linked with breast cancer.</p> <p>Texas has a particular interest in the accuracy and proper portrayal of such studies, as breast cancer is the second most commonly diagnosed cancer in Texas women. An Ecological Study of the Association of Environmental Chemicals on Breast Cancer Incidence in Texas by Y.M. Coyle and colleagues, found that twelve toxicants released into the environment by industry were positively associated with breast cancer in that state. Those twelve were Environmental Protection Agency (EPA) Toxic Release Inventory (TRI) "chemicals designated as carcinogens or had estrogenic effects associated with breast cancer risk." The release of these chemicals, furthermore, was "consistently reported to EPA TRI for multiple counties in Texas during 1988 - 2000," which included years for which the study used data.</p> <p>Some of the chemicals associated with breast cancer rates were formaldehyde, methylene chloride, styrene, tetrachloroethylene, trichloroethylene, chromium, cobalt, copper, and nickel. Styrene was the "most important" chemical linked with the disease, affecting men and women of all ages, commonly used for food storage and preparation, and released from building materials, tobacco smoke, and industry.</p> <p>The government study has been further criticized for not highlighting the many beneficial effects of better eating habits, including fewer incidences of other cancers, and improved overall wellbeing. There is no sure way to tell which tumors will grow at a more rapid rate.</p> <p>Prostate Specific Antigen (PSA) tests can help find tumors many years before they cause any prostate cancer symptoms, however routine screening of men who are at average risk is currently not recommended, because there is no proof that it saves lives.</p> <p>Prostate cancer treatments are particularly rugged, especially on older men. And many men end up after the treatment with sexual, bladder control or other prostate gland or prostate health problems. Many doctors instead recommend "watchful waiting" to monitor signs of prostate disease and treat it only if the signs or symptoms get worse. Smaller studies have given contrary views of the safety of this approach.</p> <p>This new study looked at the natural course of the disease in the men who had chosen that option. Grace Lu-Yao of Robert Wood Johnson Medical School in New Jersey was the head of the study. It's the first study involving so many older men. 50% of the men were over 75 and many of their tumors were found through the PSA tests.</p> <p>As part of this study they used the U.S. government's prostate cancer database. The researchers studied 9,017 men who were diagnosed over a period of ten years, from 1992-2002, with early-stage cancer who did not get prostate surgery, radiation or hormone therapy for at least six months. Most of the men never had any treatment whatsoever.</p> <p>Ten years later only 3 percent to 7 percent of those with the low-grade or moderate-grade tumors (which are rated by how aggressive the cells appear to be) had died of prostate cancer, while 23 percent of those with high-grade tumors died. Overall, prostate cancer killed only 10 percent of the patients.</p> <p>It's thought that most of the patients are going to die from something else and because of this most of the older men with the early-stage tumors could delay any treatment. It's not clear what should be done, if anything - whether it's conventional prostate surgery, radiation and hormone therapy or alternative treatment. This may be true even if people are younger or have a more advanced prostate cancer. Most cases are diagnosed in men who are 68 or older, and most are in an early stage.</p> <p>In any event this large study shows that a large number of men do well with no initial treatment and with no treatment long term. So this new study shows that for older men observation is a reasonable approach. Many patients do well for a long period of time with absolutely no treatment.</p> <p>To sum up, older men with early-stage prostate cancer are not taking a big risk if they just keep a good eye on the progress instead of going ahead with treatment. It's the largest study to look at this issue since the PSA blood tests to detect prostate tumors became so popular in the 1990s. It is the ninth most common cancer in the world, but is the number one non-skin cancer in United States men.</p> <p>Treatment options vary depending on the man's age, general health and stage of the cancer. Treatment may involve watchful waiting, surgery, radiation therapy including brachytherapy (prostate brachytherapy) and external beam radiation, High Intensity Focused Ultrasound (HIFU), chemotherapy, cryosurgery, hormonal therapy, or some combination. Treatments directed at shrinking the cancer often can produce significant pain relief. </p> <p>Treatments are generally given five days a week for about eight weeks. Because it may take months to years for a tumor to get large enough to feel in the breast, we screen for tumors with mammograms, which can sometimes see disease before we can feel it. </p> <p>How can I prevent breast cancer?</p> <p>The most important risk factors for the development of breast cancer cannot be controlled by the individual. There are some risk factors that are associated with an increased risk, but there is not a clear cause and effect relationship. In no way can strong recommendations be made like the cause and effect relationship seen with tobacco and lung cancer. There are a few risk factors that may be modified by a woman that potentially could influence the development of breast cancer. </p> <p>If possible, a woman should avoid long-term hormone replacement therapy, have children before age 30, breastfeed, avoid weight gain through exercise and proper diet, and limit alcohol consumption to 1 drink a day or less. For women already at a high risk, their risk of developing breast cancer can be reduced by about 50% by taking a drug called Tamoxifen for five years. Tamoxifen has some common side effects (like hot flashes and vaginal discharge), which are not serious and some uncommon side effects (like blood clots, pulmonary embolus, stroke, and uterine cancer) which are life threatening. Tamoxifen isn't widely used for prevention, but may be useful in some cases. There are limited data suggesting that vitamin A may protect against breast cancer but further research is needed before it can be recommended for prevention. Other things being investigated include phytoestrogens (naturally occurring estrogens that are in high numbers in soy), vitamin E, vitamin C, and other drugs.
